Leadership Giving Raffle

The United Way of Metropolitan Chicago Leadership Giving Incentive Drawing is organized, administered and sponsored by United Way of Metropolitan Chicago, Inc. (the “United Way of Metropolitan Chicago”), 560 W. Lake Street, Chicago, IL 60661.

1) United Way of Metropolitan Chicago Leadership Giving Incentive Drawing: Beginning at 12:00:01 AM Central Time on September 23, 2011 and continuing through 11:59:59 PM Central Time on March 1, 2012, each eligible participant may enter the United Way of Metropolitan Chicago Leadership Giving Incentive Drawing (the “Drawing”) by visiting LIVEUNITEDchicago.org/leadershipgiving and completing the online registration form. Upon submission of the online registration form, the eligible participant will receive e-mail confirmation that he or she has been entered in the Drawing for a chance to receive one of the prizes listed below and/or on the LIVEUNITEDchicago.org/leadershipgiving website. Normal internet access, phone and usage charges imposed by online or phone carrier service may apply. Limit one entry per household/family.

2) Prize Details: Winners of the drawing will be responsible for any applicable fees, service charges, surcharges and all insurance, federal, state and local taxes. Limit one prize per household/family.

3) Claiming Prizes: Winners will be randomly selected on March 1, 2012 and notified by e-mail within two (2) weeks following the drawing. The prizes will be delivered to each winner by a United Way of Metropolitan Chicago representative. Prizes will not be issued to anyone other than the eligible participant whose name appears on the registration form, and title to the property shall be taken in the name as it appears on the registration form.

If any Drawing winner cannot be located or fails to return attempts to contact him or her by 5:00pm on March 21, 2012, such winner will be deemed to have forfeited his or her prize and another winner will be randomly drawn from the remaining pool of eligible participants.

4) Winners’ List: Until April 30, 2012 individuals may request the names of the Drawing winners by sending a self-addressed, stamped envelope to United Way of Metropolitan Chicago, Attn: Leadership Giving, 560 W. Lake Street, Chicago, IL 60661 or by sending an e-mail requesting such information to leadershipgiving@uw-mc.org.

5) General Rules: By participating in the Drawing, participants agree to be fully and unconditionally bound by these Official Rules, Terms and Conditions, waive any rights to claim ambiguity in the Drawing or these Official Rules, Terms and Conditions, and further agree to be bound by United Way of Metropolitan Chicago’s decisions and interpretation of these Official Rules, Terms and Conditions. United Way of Metropolitan Chicago and each of its respective affiliates, officers, directors, agents, and employees will have no liability or responsibility for any claim arising in connection with participation in this promotion or any prize awarded. Winners assume all liability for any injury or damage caused, or claimed to be caused, by participation in the Drawing or by use or redemption of any prize or by any related activities. Acceptance of any prize constitutes permission to use winners’ names and likenesses for promotional purposes without further compensation except where prohibited by law.

United Way of Metropolitan Chicago reserves the right to disqualify false entries or entries suspected of being false, any defective, incomplete entries that do not include the required information, and/or entries submitted by ineligible participants. United Way of Metropolitan Chicago is not responsible for printing errors in any Drawing related material, or for lost, late, misdirected or non-delivered online entries, or any other failure to receive online registration forms prior to the Drawing deadline. All registration forms become the property of United Way of Metropolitan Chicago and will not be returned.

The Drawing is subject to all applicable federal, state and local laws and regulations, and is void wherever prohibited. If applicable, all federal, state and local taxes are the sole responsibility of winners.

6) Eligibility: No purchase necessary to enter or win. This promotion is open to legal U.S. persons residing in the Chicagoland area who are at least 18 years of age as of September 1, 2011. Participants must have donated, or have had donated on their behalf, at least $1,000 to United Way of Metropolitan Chicago or to its Member United Ways: South-Southwest Suburban United Way, North Shore United Way, Northwest Suburban United Way and United Way of Dupage/West Cook. Employees of United Way of Metropolitan Chicago, immediate family members and individuals residing in the same household are ineligible to participate in the Drawing.

7) Removal from Future Mailings: To have a name and address removed from United Way of Metropolitan Chicago’s mailing list to receive future mailings, simple click on the “opt out” button on e-mails or call (312) 906-2246.

8) Privacy: All personal information collected by United Way of Metropolitan Chicago will be used for administration of the Drawing. United Way of Metropolitan Chicago will not share, sell or otherwise distribute personal information collected except as stated above.
