1) United Way of Metropolitan Chicago Leadership Giving Incentive Drawing : Beginning 12:00:01 AM Central Time on September 1, 2009 through 11:59:59 PM Central time on March 1, 2010, visit www.uw-mc.org and complete the registration page by entering your name, company name, complete home address and email address Personal e-mail preferred). Once you submit your contact information, you will be entered in the drawing and notified by e-mail that you have been officially entered. Normal Internet access, phone and usage charges imposed by your on-line or phone service may apply. Limit one entry per person.

Winners are responsible for any applicable fees, service charges, surcharges, and all insurance, federal state and local taxes. Limit one prize per household/family.
3) Claiming Prizes: If you have won a Prize, you will be notified by e-mail within two weeks, following the drawing on March 1, 2010. The prizes will be delivered to each winner by a UWMC Representative.
4) General Rules: All federal, state and local taxes are the sole responsibility of winners, if applicable. In the event of a dispute regarding a prize won online, that game play will be deemed made by the authorized account holder at the time of game play. By participating in this drawing, participants agree to be bound by the Official Rules and the decisions of the judges. The Sponsor and each of its respective affiliates, officers, directors, agents, and employees will have no liability or responsibility for any claim arising in connection with participation in this promotion or any prize awarded. Winners assume all liability for any injury or damage caused, or claimed to be caused, by participation in this promotion or by use or redemption of any prize/offer/discount or by any related activities. Acceptance of any prize constitutes permission to use winners’ names and likenesses for promotional purposes without further compensation except where prohibited by law.
5) Eligibility: This promotion is open to legal residents of Chicagoland, who are 18 years of age or older as of September 1, 2009. Individuals designating through the United Way to organizations outside the Chicagoland area are ineligible to win Incentive prizes. Employees of United Way of Metropolitan Chicago, immediate family members and people living in the same household of each are not eligible.

7) Winners List: Winner’s names may be obtained up to April 30, 2010 by sending a self-addressed, stamped envelope to: United Way of Metropolitan Chicago, PO Box 75828, Chicago, Il. 60675-5828 or by sending an e-mail requesting it to leadershipgiving@uw-mc.org.
